Fairfield Memorial Hospital would like to introduce, Stephen Welty, RN, CDE, who will be providing Diabetes Education to patients at Fairfield Memorial Hospital and Horizon Healthcare with a diagnosis of Diabetes. Steve comes to Fairfield Memorial Hospital over eight years of experience as a Registered Nurse and five years as a local Diabetes Educator. As a Certified Diabetes Educator and a Certified Insulin Pump Trainer, he will skillfully work closely with his patients and their medical providers. Steve will not only educate his patients, but also can build a plan of care that focuses on healthier, reasonable lifestyle habits and will explain diabetes in an easy to understand format so patients feel efficient in managing their Diabetes.
Horizon Healthcare’s Diabetes Education Program is a Certified Program that is recognized through the American Diabetes Association. All patients, with a diagnosis of Diabetes, will be accepted to participate in the program. A Certified Diabetes Educator does not take the place of your current medical provider; however, a Certified Diabetes Educator works alongside to enhance your success in managing your health.
During an appointment with Steve, the Certified Diabetes Educator, he will focus on clearly and easily explaining Diabetes. He will then go over specific blood sugar monitoring goals, healthy and reasonable meal planning, why exercise important, and preventing or limiting serious complications from diabetes. Due to his simple, yet informative style, Steve is often able to uncover issues affecting your blood sugar that may have gone un-noticed for years. So, whether you have Type 1 or Type 2 Diabetes, or you have had Diabetes for many years or you have just been diagnosed with Diabetes, this education program will help you.
