Horizon Healthcare offering new assistance program

If you or a loved one is living with a chronic condition then you may benefit from Fairfield Memorial Hospital’s new Chronic Care Management Program at Horizon Healthcare. This program is designed to support patients who have a chronic illness by helping manage their condition. This personalized care can prevent worsening of a patient’s health condition and avoid hospital admissions.

Brenda Ware-Roge RN BSN CCM, Care Coordinator at Fairfield Memorial Hospital, is working side-by-side with the physicians, nurses and staff at Horizon Healthcare to coordinate the care needed between office visits to improve the quality of life and help patients manage medical costs. She offers patients educational tips and tools to provide self-management of their illness at home. She also has connected patients to community resources such as low cost prescriptions, transportation to appointments, home medical equipment, and area/online support groups. The program also includes a 24-hour nurse hotline to handle the after-hours urgent health needs.

“Struggling with health challenges is a trial in itself. This can make figuring out a plan to manage their health challenges and overcome other barriers extremely difficult for many. Helping patients with transportation barriers, homemaker assistance, medication management, a weight loss or disease management plans, and providing them education in various areas can make a massive difference in their ability to excel toward a better quality of life. Each patient may have a different barrier holding them back from improving their health; however, if identified, reviewed, and addressed with a solid care plan, I can support them to remove the barrier and overcome their challenges,” stated Brenda Ware-Roge, RN BSN CCM, FMH Care Coordinator.
